This Sleep Support Services Agreement (“Agreement”) is entered into between Postpartum Network, LLC (“PPN,” “Provider,” or “Company”) and the undersigned client(s) (“Client,” “Parent,” or “Parents”).
By signing this Agreement, Client acknowledges that they have read, understood, and voluntarily agreed to the following terms and conditions regarding sleep support services provided by PPN.

Sleep environment evaluations
-
In-home sleep support
-
Overnight sleep implementation support
-

-

PPN’s services are educational and supportive in nature and are not medical treatment, therapy, behavioral health treatment, or a guarantee of specific sleep outcomes.
2. NATURE OF SLEEP SUPPORT & TRAINING
Client understands and acknowledges that:
-
Sleep development varies significantly among infants and children
-
Sleep training and behavioral sleep shaping may involve periods of crying, fussiness, protest, disrupted sleep, frustration, or temporary regressions
-
Some sleep methods may be emotionally difficult for parents
-
Results depend heavily on consistency, developmental readiness, temperament, illness, feeding patterns, environment, and caregiver participation
-
No specific sleep outcome can be guaranteed
PPN does not require or force any specific sleep training method. Recommendations are educational in nature, and parents maintain full authority over implementation decisions.
Client may discontinue implementation of any recommendation at any time.
Client understands that crying is a normal form of infant and toddler communication and protest during behavioral change and routine adjustments. PPN does not guarantee that infants or toddlers will not cry during implementation.
3. DEVELOPMENTALLY APPROPRIATE EXPECTATIONS
Client understands:
-
Infants and children develop sleep skills at different rates
-
Night wakings may remain developmentally appropriate
-
Some infants may continue to require overnight feeds
-
Teething, illness, travel, growth spurts, developmental leaps, vaccinations, separation anxiety, and regressions may temporarily disrupt sleep progress
-
Sleep training does not guarantee uninterrupted overnight sleep, independent sleep, nap success, or elimination of all night wakings
4. CLIENT RESPONSIBILITIES
Client agrees to:
-
Provide accurate, truthful, and complete information regarding the child’s medical history, feeding history, sleep history, medications, developmental concerns, diagnoses, routines, and household circumstances
-
Inform PPN of any medical concerns including reflux, respiratory concerns, allergies, feeding difficulties, poor weight gain, seizures, developmental delays, behavioral concerns, or other conditions that may impact sleep
-
Consult the child’s pediatrician regarding any medical concerns
-
Maintain reasonable consistency with agreed-upon routines and implementation strategies
-
Ensure all caregivers attempt consistent implementation whenever possible
-
Maintain safe sleep practices
-
Participate honestly and consistently throughout the process
-
Provide timely communication and follow-up when requested by PPN
Client understands that incomplete, inaccurate, or withheld information may negatively impact the effectiveness of services.
Client further understands that inconsistent implementation may significantly delay or prevent progress.
5. MUTUAL EXPECTATIONS OF SERVICES
PPN agrees to provide sleep support services to the best of its ability using professional experience, clinical knowledge, evidence-based sleep education, and information provided by the Client regarding the child, household, parenting preferences, routines, lifestyle, and goals.
Recommendations and guidance are developed using:
-
Information provided by the Client
-
Developmentally appropriate sleep expectations
-
Evidence-based sleep education
-
The child’s temperament and needs
-
The family’s parenting style and comfort level
Client understands that successful sleep training requires commitment, consistency, structure, and caregiver participation.
In return, Client agrees to:
-
Follow agreed-upon sleep schedules, routines, feeding guidance, and implementation recommendations as consistently as possible
-
Maintain communication with PPN during implementation
-
Ensure all caregivers understand and follow the agreed-upon plan
Client understands that inconsistent implementation by parents, relatives, babysitters, nannies, grandparents, or other caregivers may negatively impact sleep progress and outcomes.
6. PROGRAM READINESS & IMPLEMENTATION REQUIREMENTS
Clients should only begin the sleep training process when they are able to dedicate approximately fourteen (14) consecutive days and nights to consistent implementation of the sleep plan, routines, schedules, and recommendations provided by PPN.
During this implementation period, Client agrees to:
-
Follow recommended sleep schedules, routines, feeding schedules, and implementation guidance as consistently as possible
-
Minimize unnecessary disruptions to the child’s routine
-
Ensure all caregivers are informed of and committed to following the agreed-upon plan
-
Maintain consistency across all caregivers and sleep environments whenever possible
PPN strongly recommends delaying the start of sleep training if the family anticipates:
-
Travel
-
Visitors or family staying in the home
-
Holidays
-
Schedule disruptions
-
Daycare transitions
-
Illness within the household
-
Vaccinations
-
Major developmental transitions
-
Significant routine changes
-
Any event likely to interfere with consistency
PPN recommends families “clear their calendar” as much as reasonably possible during the initial implementation period to allow for maximum consistency and success.
For overnight and independent sleep-focused programs:
-
The child must sleep in their own designated sleep space
-
The sleep environment should remain consistent throughout implementation
-
The child should generally be at least ten (10) weeks old and/or approximately eleven (11) pounds unless otherwise approved by the child’s pediatrician
-
The child should be generally healthy and medically appropriate for participation
If Client has any concerns regarding developmental readiness, feeding needs, medical conditions, weight gain, prematurity, respiratory concerns, or overall suitability for sleep training, Client agrees to consult the child’s pediatrician or healthcare provider prior to beginning services.
7. PEDIATRICIAN CLEARANCE & MEDICAL DISCLAIMER
